So how does HealthWEST help me and my family?
HealthWEST is a charitable organisation whose aims
are:
- to improve the health status of the enrolled
population of West Auckland
- to reduce disparities between the health of different
groups within the population of West Auckland
- to promote and develop a fully integrated health
delivery system
- to promote good health and the prevention of
disease
- to support the delivery of quality health services
through a skilled workforce
- to reduce barriers to people accessing primary
health services, and
- to work with other health providers within the
region to ensure that services are co-ordinated around the needs
of the enrolled population
HealthWEST is progressing toward these aims. Here
are some examples of what we are currently doing:
HealthWEST member practices provide free diabetes checks for their
diabetic patients to help improve and monitor their condition.
All HealthWEST practices have free access to on-site or telephone
interpretation services in over 50 languages.
Patients of HealthWEST practices under 22 years of age can access
free sexual and reproductive health consultations.
HealthWEST operates free GP clinics within low decile schools
at no cost to the student or the school. These clinics are currently
available at Henderson High, Kelston Girls High, Kelston Boys, Massey High and Waitakere College.
HealthWEST practices can provide free and reduced cost nutrition
clinics for those holding a Community Services Card or a High User
Health Card and for those of Maori or Pacific Island decent.
The Waitakere Palliative Care Project provides a high quality, timely service that embraces the principles of the New Zealand Palliative Care Strategy 2001.
This service assists patients to remain at home with high quality care in their terminal illness. It provides up to ten home visits and five surgery consultations free (with your family doctor) to patients and their families/whanau. The patient’s own family doctor provides palliative and terminal care services (at home or in surgery), with the support from West Auckland Hospice and/or the District Nursing Service.
This service is available for all patients and their families/whanau in the West Auckland region. It will be responsive to the varying needs of patients during the stages of their illness. Patients and families/whanau are encouraged to access care when the need arises in their home.
All contributing GPs and nurses will have access to specialist support and advice.
This project is being evaluated for ongoing funding and we will value your feedback.
If you would like to receive this Palliative Care support, please do feel free to discuss this with your GP.
